Personal Information
Your personal information is your name, phone, address and email. We use and disclose it only in order to contact you and perform services you request. We do not share or sell personal information. Period.
Health Information
Your medical-related health information has separate legal obligations from your personal information. We use and disclose it only as detailed below:

Treatment: We may use and disclose health information to another physician or healthcare provider who treats you.

Payment: We may use and disclose health information to obtain payment for our services.

Emergency: In the event of your incapacity or in emergency circumstances, we may disclose your relevant health information to people who are involved in your care.

Reminders: We may use or disclose health information to provide you with appointment reminders in the media you indicate, such as voicemail messages, postcards or emails.

Authorization: We may disclose your health information to a family member, friend or other person to the extent necessary to help with your healthcare or with payment, but only if you agree that we may do so. You may give us written authorization for other purposes, which you may revoke in writing at any time.
Patient Rights
Access. You have the right to review or receive copies of your health information, with limited exceptions. You must make a request in writing (email is acceptable) for access to the health information of your child. You may request that we provide copies in a hard-copy format, which we will provide unless we cannot practically do so.

Alternative Means. You have the right to request that we communicate regarding your health information by other means or to other locations with a request via email or in writing that specifies the means or locations.

Amendments. You have the right to request that we amend your health information, with a request via email or in writing that explains why the information should be amended. We may deny your request under certain circumstances.

Disclosures. You have the right to receive a list of instances for the last 6 years in which we disclosed your health information for purposes other than treatment, payment, or activities you authorize. If you request this more than once in a 12-month period, we may charge you a fee.

Restrictions. You have the right to request that we place additional restrictions on our use or disclosure of your health information. We are not required to agree, but if we do, we will abide by our agreement except in an emergency.

Changes & Termination. We reserve the right to update, delete, or otherwise alter this website without notice, but have no duty to do so. We may terminate or discontinue this website or any part of it at any time, for any reason, without notice or limitation.

All content, including but not limited to headlines, copy, “look and feel,” navigation, photography, audio, images, demos, illustrations, software, text and video clips, are the property of CCD. As protected under US and foreign copyright, trademark and other intellectual property laws, this content may not be used without our written permission.

IP Protection. We will never knowingly plagiarize another’s IP. Please advise us of possible infringement by emailing with your name, description and URL of the alleged original work, your contact information and a statement that you are the work’s owner or agent.
